DHL Boeing 757 crash in Costa Rica Explained By an Airline Pilot (First Look)

A DHL Aero Expreso Boeing 757-200, registration HP-2010DAE performing flight D0-7216 from San Jose (Costa Rica) to Guatemala City (Guatemala) with 2 crew, was climbing out of San Jose's runway 07 at about 09:40L (15:40Z) when the crew stopped the climb at about FL210 declaring Mayday due to a hydraulic failure. The aircraft entered a hold while the crew worked the related checklists. The aircraft subsequently returned for an approach to runway 07 about 45 minutes after departure. After touchdown at 10:24L (16:24Z) the crew lost control of the aircraft, which spun around and went off the runway causing the aircraft to break up into two parts, both main gear struts collapsed. The crew was able to exit the aircraft without serious injuries.

  • Hi Denys! Great analysis! Having many thousands of hours in the 757 I will be curious to see what information they get from the flight and voice recorders. The checklist procedure for any loss of hydraulics is a Flaps 20 landing - primarily to ensure that a go around could be accomplished. If you were at full flaps and unable to raise the gear or flaps, a go around might not be possible. When we practiced hydraulic problems in the simulator, it always seemed like they would fail the left system, just because there was so much on it, including the nose wheel steering. The plan was ALWAYS to just stop straight ahead. Not sure why Boeing didn’t build a bit more redundancy into that system, but like you said, this is a fairly old design. Thanks for the great analysis and I guess we’ll all look forward to more information once the investigation is complete. I’m sure the pilots will be able to provide a lot of information as well!     • The drop-off definitely didn't help. Having something to slow down at same elevation would have been better although I am curious as to why the runway was higher anyway, doesn't seem like its on the side of a hill cause both sides of the runway seem to be lower.

  • Hello Denys, excelente vídeo and congratulations, I’m listened to the conversation between the controller and the plane and is true, they lost the left hydraulic system. And about the position of the flaps, if you extend in alternate mode, is posible extend maximum 20 degrees, this is the reason the plane land with higher speed.

  • So they have tail wind 8-10kts, less than ideal flaps, and I think you're right. It's possible that they engage reverse thrust on both, but the left didn't open/deflected, so it produce forward thrust instead. I think this is still a good outcome, crew is safe, plane didn't blow up/ catch fire.

  • Hi, great video. I am not a pilot or profesional in aeronautics, just a big fan of airplanes. I am from Costa Rica, SJO airport has runways 25 and 7, the best landing runway is 7 because the terrain in the approach is low as you come from the ocean, but the issue here is our airport is 3000ft high and the winds change a lot. Runway 25 is very hard, the terrain and physical obstacles begin at 2 km from the end of the runway so takeoff and landing routes are actually turning south because of this issue. Hope my information is helpful.

  • Hard to tell from the low quality video but it looks like the port side engine thrust reversers didn't open so you have reverse thrust on the right and forward thrust on the left. That would explain the yaw. It looks like they didn't know the reversers weren't both working so gunned both engines to try and bring the plane to a stop. By the time they realised, it was too late.

  • Good catch 5:00 trust asymmetry...I have seen other pilots analysis but none have a clue why the plane did almost a 180º turn. If the brakes are good (no lock,just smoke for being too hot) I think the way the plane turns is really weird,but if the left engine was pushing the plane it is the only way you can explain this plane going off road to the right. They made a U turn to the ditch,almost like a handbrake turn. Maybe they did not realize about Left TR fail and pilots expected another thing. If you had a fail in one of the engine TR would you use the other engine to brake or brake only with the wheel brakes to avoid trust imbalance?

  • I have landed there several times. After a while, I just refused to go there because it is such a dangerous airport for many reasons. It is a "Special" airport. RW 7 is almost always used for landing - even with a tailwind because of the mountains. RW 25 is the preferred RW for takeoff. Most of my landings there were at night with weather down to minimums. I always just barely got in. The go-around is involved and scary. Also, there is no close alternate. The runway is not grooved, so if it is wet, good luck stopping. One night I had max reverse, max brakes, and it was not slowing down at all for a long time. This airport is the last place I would go with a hydraulic problem. I think the pilots are going to get blamed for this accident in the end. If they had PAX, there would have been deaths.

    • For me it´s the question why they didn´t diverte to Liberia/ Costa Rica or Panama-City Tocumen. Both airports are much easier to approach and Tocumen is even the main base of the airline. Surely they had enough fuel on board, the original destination of the flight was Guatemala City.

    • @@NicolaW72 One thing to keep in mind, cargo carriers always fuel for max payload, min fuel. I fought that battle many times. Most pilots just give in to the company and land on fumes.

  • Denys, thank you for this great analysis. Is it possible on the 737 to unintentionally deploy only one thrust reverser? Asking to figure out why only one deployed. Great catch on the smoke being pushed by the number 1 engine, but not the other. The thrust asymmetry does make sense for the drift. Pity that the gully was so deep. If it were flat, they might have been able to save the aircraft rather than having to write it off. Great job on this video!

  • The 757 is only 5 years older than the A320, and the systems powered off of each hydraulic channel seem to be pretty similar between the two types (nosewheel, LH reverser). I can't say I agree with your comment, especially considering the 737 NG still retains the basic 1967 hydraulic system architecture from tne 737-200.

  • From behind you can see only half the spoilers are up. Unless the cause of the leak was in the line between landing gear actuation and nosewheel steering in the left hydraulic system they should have had nosewheel steering powered through the PTU. Automatically turns on when left system pressure low. If engaged it will power Slats, Flaps Landing Gear and Nosewheel steering. But it is no proof since Flaps can also extend electrically (ALTN FLAPS) and Gear can lower through gravity as you said. I think the cause here is a heavy airplane (happened after takeoff and they returned immediately) and the tailwind on landing plus only one reverser operative and only half of the spoilers. Why it spun we can only guess...Thrust assymmetry? Has nothing to do with the hydraulic failure so why did that happen? two catastrophic failures at once? I doubt it! Nosewheel steering system inop due to leak was there? could be... You can see the Rudder is at full left at the beginning of the spin. Maybe the pilot used rudder only for directional control and forgot to use the steering tiller at lowering speed because of the stress? It is not clearly visible if the nosewheel is turning but a rudder deflection can also come from the steering tiller use. Another question that comes to my mind is: Why did they brake so hard? panic? There was still ample runway remaining approx 950m at the point of spinning (exit Bravo). No need to force braking. Also, if you force braking you don´t have differential braking effect, do you?

  • I'm an aircraft mechanic.. The Main landing gear shouldn't be smoking. It appears that the ABS aren't working or disabled. In planes ABS is needed, or all the main tires could be locked up and blown in a skid like you see in the video. Planes have true power brakes. You cant feel if the wheels are locked and skiding. I have seen this before. Pilots forgeting to engage the ABS. They blew four mains and grinded the tires and the wheels down. They shredded half the rims and even the brakes. The DHL incident looks like that. The braked were working on the DHL plane. Looks like no ABS.

    • If you are an aircraft mechanic...which I don't believe...you should be familiar with very common aviation nomenclature and concepts: The system that prevents a skid and loss of control in aviation is called "Antiskid", not "ABS". Tires do not "blow" on aircraft wheels. That is because the wheels have fuse plugs that melt at a predetermined temperature. This allows for more gradual release of the nitrogen, contained within the tires. Controlled release of the nitrogen from the tires prevents them from exploding and causing aircraft damage, especially to the wings, which contain fuel.

    • @@Flyfisher1976 I'll write so that everyone can understand the concepts. I'm not a jerk like others. Antiskid and Abs works the same way!!! Preventing skids.... or locking up the tires. Means the same thing. Planes with hydraulic braking hsve full power brakes. You arent going to feel the wheels locking up. And so, if the wheels lock up on a heavy plane the wheels will grind. I have seen this. The plugs can blow , the tires will loose pressure but, the wheels will remain locked if the pilot is still applying pressure. The wheels will grind. If i want people to understand, I'm not going to write in a manner so that people don't understand. I have to break it down and not be a dumb ass. Apparently thats what you want to be.

  • SJO airport is really hard for landing in Rwy 25, and it has displaced threshold and less than 8000ft, for that reason is very common use rwy 07

  • Think they were screwed when their tires blew out and they began sliding on the wheels only. That is my determination of the probable cause of the runway runoff. All in all a very good job by the crew. Does Costa Rica have an NTSB? If so they will investigate for 2-3 years and then a stupid probable cause will be published and no recommendations for changes will be made. I hope Costa Rica does a better job than we have come to expect from our NTSB.

  • I understand that the tyres were not burst and were still on the wheels because we can see the black skid marks right up until the plane left the runway, but isn't that a problem in itself? Why were the brakes and wheels locked enough to skid and leave tyre marks in the first place? I'm not a commercial jet pilot, so please excuse my ignorance, but don't large jets have sophisticated anti-lock/anti-skid systems on their brakes? If the anti-lock/anti-skid system was working, the wheels would be turning just enough to provide maximum retardation, but not locking and leaving black lines along the runway. Could a separate hydraulic failure have disabled the anti-lock/skid system? Once the tyres were skidding (producing those huge plumes of smoke?) the pilot wouldn't have had much directional control of the aircraft, and the loss of the left engine's thrust reverser would only have made things worse. It would have been like aquaplaning or driving on ice. Is it possible that the pilot could feel that the brakes were not 'biting' properly, or knew that they had locked solid, making the plane skid to the right, and made the decision to try to direct it along the fast taxi-way deliberately to make the best use of the already crippled systems left to him? With working (not locked) brakes and a symmetrical engine configuration, I imagine he'd have continued along the main runway. Perhaps the sharp right turn was an attempt to make the best of an impossible situation. No disrespect intended to anyone concerned; I know that speculation like this is stupid and wrong, but sometimes it can help other people see things from a different perspective.

    • That is the good point. Actually I don’t know if the wheels were locked. I think they were still rotating. It other case they would lost them very fast. I think they stepped on the brakes… Probably there could be problems with anti skid but it should have been working even from hydraulic accumulator that is a part of the standby system. Well, I can’t 100% be sure about it but you may have the point. Thanks for your awesome comment❤️✈️

  • I believe the pilots panicked, applied hard differential (or full braking), while attempting to take the high speed turnoff. My opinion: The pilots were convinced that they were out of useable paved surface to come to a safe stop, when they saw the displaced threshold markings approaching. In fact, looking at 17:40 you can see a white diamond. From that white diamond there is **STILL 3000' OF USEABLE PAVED SURFACE REMAINING** to bring the aircraft to an expedited, but safe stop. We know by the marks on the ground, that all four wheels were locked up and the plane was in a skid. The pilots had most likely lost directional control at that point. Did they intentionally turn off antiskid and apply differential manual braking, in an attempt to stop at what they THOUGHT was the end of the runway, losing control or attempting to make the high-speed turnoff? This may have lead to complete loss of directional control, causing the sudden runway excursion. Was antiskid disabled, inoperative or somehome compromised by the original hydraulic failure?

  • Denys, thank you. What was the function of the area that destroyed the plane? Why wasn't the surface there level with the runway? On level ground the plane would have turned but not broken up. Perhaps the airport should also be reviewed to improve things.

    • Someone in another Video about this incident said, it was a region with lots of rain so they had to design the airport like this too get the water out of the way. 10cm of water on a runway would be chaos.

  • Question. Doesn't the Boeing 737 still have some mechanical link from the controls in the cockpit to the control surfaces on the wings and tail? I think it's the only large jet that still has such a link. I think Boeing calls them "Servo tabs" or "Balance tabs" maybe.

    • Serious guess from me would be they are a part of the rainstorm water drainage system. I've seen references to "drainage ditches" or "open drainage ditches" at airport in the context of runway expansion work, so I assume it is exactly what was referred there.

    • @@supercat380 I would disagree. Water needs to get off the runway fast and it is not an easy thing with the runway sizes and on almost horizontal surface. As an armchair expert on the matter I suspect poor braking and steering control as a common occurrence with a risk of hydroplaning would be much worse than any extremely rare skid off events. It is fairly common to read aviation news where stormy weather disrupts airport operations, forcing many planes into holding patterns to wait for the brief openings or divert outright. Looking at FAA advisory I see that 3mm of water depth is defined as the line between runway being classified as wet and as contaminated. It would depend on specific aircraft which depth is considered dangerous, eg for B737-800 I see reference to 13mm of water as maximum depth for takeoff or landing. Not even that deep, really. I am not involved with aviation industry and my opinion is about as fact based as that of any other random dude on the internet. However what I admire about aviation industry is how safety focused it is. I trust should it be determined an actual safety concern based on hard data available to the experts that authorities would order to ditch the ditches in favor of some other alternatives.

    • @@Xerkus good constructive and logical points you made here. However, many airports are quickly adapting to improved and revised water drainage to assist in quick dispersal of large volumes of water which may endanger aircraft during severe rain and storms. Hydroplaning has been a major contributor to a significant number if runway- based accidents and the problem is been taken seriously by aviation authorities. Your points are indeed very welcome and very informative!
