What's the Best Position to Sleep in? Do we even need a Pillow?

Is sleeping position that important? Would we be better off without a pillow? The science addressing these questions is lacking, but we can still make some conclusions based on the research.

CORRECTION: It’s no longer accurate to say the brain *doesn’t have a lymphatic system… [Nature article from 1 day ago - go.nature.com/2SE2pkL] The central nervous system (CNS) was long thought to lack lymphatic drainage; fluid and macromolecules were instead thought to be cleared from the CNS by other routes, such as through absorption into the bloodstream, or through channels running along the outside of blood vessels or nerves to reach the lymphatic system outside the CNS.
As for the sleep experiment, I'm thinking of doing:
Week 1 - Sleep on my bed with a pillow. I will try to fall asleep on my side and will try not to use the pillow to lay on my back.
Week 2 - On bed, no pillow.
Week 3 - On floor with padding. Whether I make the pillow available or not will depend on on the results from Week 1 and 2.
*I'll aim for at least 5 days of data per week to have some leeway for if I go out and the results are skewed by alcohol or what have you.

Maybe do a before and after pushing air through a straw. (Basically to try an keep the output consistent) See how much longer you can breath out. • I've been working with patients for years and the easiest answer to sleeping on your side is to utilize a supportive body pillow. This actually accomplishes many things. By having your top leg atop the pillow, you're keeping your hips in a more neutral position which can decrease pressure and pain in that region. Similarly, by having your upper arm on the pillow, you're allowing your entire shoulder -- particularly your scapula -- to rest in a more useful position. Whether you use a pillow under your head or not is relatively unimportant. After a short period of sleeping with a body pillow, you'll find that you carry it with you even if you flip from left to right side throughout the night.

Something I learned along the way is the passages of the nose. When sleeping on either lateral side the side of the nose closest the ground closes and the other opens making it easier to breath on that side. When I'm on my back both passages of my nose are obstructed forcing me to breathe out of my mouth and snore.

    • The Prophet SAWS (peace and blessings of Allaah be upon him) used to sleep on his right side, putting his right hand on his right cheek, and this is what the Prophet SAWS (peace and blessings of Allaah be upon him) recommended others to do. According to a saheeh hadeeth narrated from al-Baraa’ ibn ‘Aazib, the Prophet SAWS (peace and blessings of Allaah be upon him) said: “When you go to bed, do wudoo’ as if for prayer, then lie down on your right side…” (Narrated by al-Bukhaari, al-Wudoo’, 239). The Sunnah is to put one’s right hand under one’s cheek. It was narrated that Hudhayfah (may Allaah be pleased with him) said, “When the Prophet SAWS (peace and blessings of Allaah be upon him) went to bed at night, he would put his hand under his cheek, then say, ‘Allaahumma bismika amootu wa ahyaa (O Allaah, in Your name I die and I live).’ Then when he woke up he would say, ‘Al-hamdu Lillaah alladhi ahyaanaa ba’da ma amaatanaa wa ilayhi al-nushoor (Praise be to Allaah who has brought us back to life after causing us to die, and to Him is the resurrection).’” (Narrated by al-Bukhaari, 6314). Ibn Hajar said: The right side is singled out for a number of reasons, such as: this lets a person wake up more quickly; it also allows the heart to lean towards the right hand side. Ibn al-Jawzi said: the doctors have stated that this position is more beneficial to the body. They said to start by lying on the right for a while, then turn over to the left, because the former aids peristalsis and sleeping on one’s left side facilitates digestion. Al-Nawawi said: this hadeeth mentions three acts that are Sunnah, the first of which is doing wudoo’ before going to sleep. If one already has wudoo’, there is no need to repeat it, because the aim is to go to sleep in a state of tahaarah (purity). The second is to sleep on one’s right side… This indicates that sleeping on one’s right side is Sunnah, and if a person does that to follow the example of the Prophet SAWS (peace and blessings of Allaah be upon him), he will be rewarded for that. Sleeping on one’s left side is permitted, but one misses out on the reward for following the Sunnah. And Allaah knows best.

  • I noticed that the most comfortable position for me to "start" sleeping is on my left, face towards the wall. On the other hand, the typical reading in bed in the evening is more comfortable on my right. So I can actually just switch by rolling around to switch form one to the other. That works pretty well. Sleeping on the side is also surprisingly comfortable even on harder surfaces, like when out camping or on the ground.

That way, I can comfortably rest both legs on top of each other and prevent my spine from twisting - A bigger pillow I can rest my left arm on (essentially like a spooning position), so the arm won't just hang down and I don't hunch over with my shoulders.

    • What about all those 'beauty gurus' who say that sleeping on the back prevents skin aging (because there's no friction and folding like when you sleep on your side\stomach and your face sinks into the pillow). It's also impossible for me to sleep on my back, but that's such an easy thing to slow down skin aging, seems perfectly logical. Also everyone praises silk pillow cases, I bought a pair, but I still wake up with creases on one side of my face, it's infuriating. It's like wrinkling is the price one has to pay for comfort and disposal of physiological waste products from their brain. Of course it's a health channel, not a beauty one, but I think people over 30 could relate to this. Waking up with the face resembling a crumpled plastic bag slighty affects your wellbeing.
