  • Love your content, but want to clarify some terminology. When it comes to bones, fractures and breaks are the same thing. Fracture is the accepted medical term. There are different types of fractures. nondisplaced (the two ends of break are still aligned and close to each other. Usually heals with immobilization (eg cast or rest in the case of rib fractures Displaced (the ends have moved away from one another and typically require manipulation or surgery to bring them together). Comminuted means multiple pieces of bone, and compound (the skin is broken and risk of bone infection is high). In Wout’s case, we hope his fractures are nondisplaced. The other concerning thing is that he fractured his sternum. This typically takes a great deal of force and raises concern for other deep tissue or organ injury - which may mean a time longer time to get back on the bike

    • I was thinking Wout's rib fractures might be nondisplaced because he didn't have a collapsed or punctured lung, but I know that is not necessarily accurate. Remembering back to Julian Alaphilippe's injury he had rib fractures and also a hemo pneumothorax, which I think is a punctured collapsed lung with blood in the chest cavity, (correct me if I'm wrong) and a fractured shoulder blade and it took him a long time to just come back.

  • Belgian Doctor's say its quite impossible to go to the giro. here in Belgium it's huge news. His recovery will be longer than expected, even at the tour he will not be at his best

  • A SX rider broke his collar bone, had surgery and came back, and raced in 2 weeks. And placed in the top 10, but like you said ribs suck I cracked 2 ribs, and that was the most painful injury I've ever had. I wish Wout and the others a speedy recovery. Breathing is going to be the hardest thing to do for Mags. I hope Jorge can hang with MVDP.

  • Hey Chris, your medical assessment is pretty spot on, but as a Dr who has spent a few years doing orthopaedics, I would only pick you up on a little semantics relating to the perennial ‘fracture’ vs ‘break’ argument. For me they are synonymous. The distinction to make is whether the ‘crack in the bone’ (whether you call it a fracture or break) is displaced/ rotated or not. An undisplaced fracture will, all other things being equal, heal quicker than a displaced fracture. Interestingly, fractures heal better when there is a little movement at the site, which happens when the affected limb is held in POP (plaster of Paris). When the fracture is completely immobilised by either internal or external fixation (e.g. a plate) while the patient can generally mobilise more quickly, the fracture itself takes a bit longer to fully heal. Additional complications, such as your punctured lung, or whether the fracture is ‘compound’ (I.e. the bone spicule breaks the skin, can delay the healing process. The fact that WvA is a trained athlete and was on top form is in his favour for a swift return, let’s hope so.

  • I hope van Aert's sternum is not broken and displaced.Having had that happen to me ( car accident, chest part of seatbelt impact ) , that is months of recovery. As for broken ribs ,well having broken two in my back close to my spine ,let's just say there's a six week period when you're scared to death you might sneeze or fart . Those are both bad news. Even without displacement or location issues , my guess is that the Giro is best case scenario with return to fitness after being recovered enough to load those bones. Wish Wout the best case scenario and a speedy return to form. Same for the other injured riders.

  • Note of clarification a break is the same as a fracture. The real issue is how much energy is applied to the bone. Imagine a wine glass wrapped with duct tape. If you drop it from ten feet to a hard surface the glass will probably be broken or fractured but still look like a glass as the tape holds all the pieces together. Bone is the same brittle but covered with tough connective tissue called periosteum. If there is sufficient energy to just break the bone you have you have a non displaced fracture. If there is more energy enough to tear the periosteum you have a displaced fracture. If the sharp ends of the bone have enough energy they can damage surrounding soft tissue things like lungs or some times vessels. Wouts injury is severe just by counting the number of bones broken. He will be lucky to ride the tour.

  • Thanks for the update, Chris. Sorry to see any rider in pain, but seeing Wout like that made me wonder how long it would take me to even look at my bike after a crash like that. It took 3 weeks after my hip replacement to look at it, 4 to get back on and ride.

  • The Belgian sports media is not as positive in its forecasts for Wout as you are, Chris. The general consensus is that riding the Giro is actually impossible for Wout. After all, it is really assumed that those ribs are broken and not cracked. According to the doctor of the Belgian Cycling Federation, it would even be dangerous to restart training too early because if rib fractures are not sufficiently repaired, there is too great a risk of complications such as a collapsed lung and the like. I personally think that, at best, he can aim for restarting full training at the beginning of June and then he should aim for building up through smaller races towards the Olympic Games and the Vuelta, which he would normally also race for the first time this year. ...but if complications arise, the Olympic Games may also not be feasible. Worst case scenario, you will see him back in the late summer months for the summer and autumn competitions such as Plouay, Hamburg,... Best case scenario, he can restart competitions at the beginning of June. It is also assumed in Belgium that the Tour will still not be on the program after this fall because the Olympic Games were/are the main goal this summer.

    • Doctors never once told me I could train right away after I crashed and fractured my ribs. They never want to be sued later and always go on the caution side of rehabilitation. But if the ribs are only fractured and stable you can always train on a trainer very soon if not right away. Outside training is too much for sure but then maybe ten days only is needed for easy non racing training. If we don’t see Wout training right away we know he has a major break. 🤔🦋🤞🙏

  • I think VLAB is underplaying it. Sternum fractures (unless it’s like a small xiphoid issue) take a lot of force. Usually a direct blunt trauma to the chest. If you look at the heli footage, it’s so hard to tell, but I think he went over the bars and chest down onto another bike. The problem is, if you hit hard enough to fracture it you can do some lung or heart damage as well. A few non adjacent Displaced or non-displaced rib fractures only, he goes to the giro, but I say no way Giro on any good form, but TDF is gonna be bangin!!!

  • I cracked a couple ribs last year in a fall riding outside in the Winter and for the first couple weeks I just prayed that I did not cough or sneeze as it was so painful to take a deep breath or even turn my body. I was off the bike for almost 6 weeks but I took it easy, I probably could have started again a week or two before but the weather was still cold. Lets see how long it take Wout to recover..........

    • Dude, so unbelievably painful. Pain I didn't know existed. The only thing that comes close is a herniated disc I had. My kids try to make me laugh because it makes them laugh when I start crying from the pain.

  • Real life trrauma surgeon here. To be picky, a fracture and a complete break are the same thing. What matters is the degree of instability that results. Since the time you were racing, Chris, we have now developed ways to plate ribs and the sternum. I have no idea what was done with Wout, but it is possible that he was cobbled back together which should help him recover more quickly.

  • Excellent insights Chris. I think re Jasper Philipsen, his most important job will be in the early race where the organisers have said they have chosen wider roads than usual in an attempt to make the racing safer - I think we all know it will just make the racing quicker, and Alpecin will appreciate having someone with real afterburners who can put MvdP exactly where he needs to be even if the bunch is flying along at 60-70kph

  • Chris - a fracture IS a break. What you mean is does Wout have a a displaced fracture or not. Non-displaced fractures are what you are talking about regarding WvA. However, you describe displaced fractures when you went down and your lungs were punctured.

  • Wout has already been scratched from the Giro as per the PCS Rider Updates. It may be possible for him to ride in the Tour, but it's one thing to participate and another to be at the same level of fitness that he was prior to the crash.

    • I’m implying that Jonas hasn’t won a Tour without Wout on the squad. I’m implying that Wout turns himself inside out stage after stage which drastically increases Jonas’ performance and sucks the life out of the other teams. I’m implying that while Jonas maybe externally bummed about Wout’s crash, internally his probably high five-ing himself as Wout is all but guaranteed to now ride the Tour which caused the rest of the Tour peloton to externally and internally let out you’ve got to be f’ing kidding me. Wout is a beast and from a team standpoint they need him more than he needs them. While Jonas is getting paid fat stacks I’m implying that if I was Jonas I would and would’ve had a conversation with Wout before schedules were announced where I’d offer some extra Euros if he rode the tour, whether that personal dollars or sponsorship team up outside of the team. Prior to this crash Jonas was heading to the Tour without Primo and Wout. It would be foolish to think that those moves wouldn’t negatively impact him. Add in Kuss hasn’t yet returned to top 10 form yet. Stress level in getting high and Wout going to the Tour would bring a sense of calm.

Wout is fantastic and he was really crucial on the coubble stage(i think stage 4 or 5) tour 22, when he gave up a probable stage win for himself and helped Jonas not to loose too much time to Pogi after a mechanical problem changing bikes. Tour 22 he was crazy strong. But then Jonas gave him a stage win back in the last ITT tour 22. (One of the only times wout has cried, when he got gifted the stage by Jonas) (Thats helping each other in nice balanced way on a team)Dont get me wrong. Wout is fantastic. I love him and of course he makes any team on the planet much stronger. But to say that Jonas Tour wins would not have happened without wout is in my opinion wrong. Jonas is just so much stronger the anybody in week 3 in a GT, that he without any accidents right now will win 9 out of 10 GTs with mountain stages or hilly ITTs placed in week 3. eventually a new young phantom will ariive on the scene(maybe ayuso in a few years), but instill then forget it. He will win without incidents every GT he will attend instill then. With or without, Wout, Roglic or KUSS. Jonas bloomed late(in my opinion because of low self esteem). and he is still getting stronger His only weaknees in my opinion is his humble nature, being too nice., not greedy enough Gifting a Grand tour away. Ohhh my god. Thats just stupid. He could learn alot from Pogi being hungry for victories. hopefully Wout will come back stronger and most important win a Monument(he derserves that and should have had one already, my bet not being belgian, is that he biggest dream is the flanders, so lets hope next year. Pogi fans forget one thing(im actually a big Pogi fan myself) Pogi knows that he cannot win against Jonas in week 3, without any incidents. Jonas has won in week 3 all 3 tours they have completed against each other. And of course being Pogi you cannot have 3 years in a row without a GT win, and he knows that his chance in the tour and the vuelta where Jonas attends is going to be very difficult, so of course he will choose The giro, evverybody would and with the rooster this year. He got that one in the bag of palmares. He is the best road racer right now on the planet, but Jonas is by far the best stage racer on the planet right now. He just do not like too much attention and media. But believe me, he has the potential and with no bad luck to get 8-11 GT wins and then be one of the best stage racers in history. My prediction for Jonas 5 tours, 3 vueltas, 1 giro. My prediction for Pogi. 3 tours(it would be great if he wins one more late in his carreer. 3 giros and 1 vuelta. He will in my opinion try to win every big race on the world tour. I really think this is his goal iinclusive the olympics and the rainbow wc of course, and he will probably make it. Its exiting times to be a cycling fan. But make the races more safe for the raiders please.

Then Jonas gifted him the stage win the ITT stage 20 tour 22. Wout cried on camara for one of the few times in his life in gratitude for Jonas gift to him. Thats balanced team work. and gifts) Wout was a unbelievable beast tour 22. tour 23 not so much for his standards. The fact is that Jonas is just that much stronger in week 3 than anybody else. And he has and will win the tour with no bad luck or accidents with or without wout, Roglic or Kuss for that matter. Tour 24(again with all healthy and no incidents. If we are lucky it will be close instill stage 19, then Jonas will destroy everybody by minutes in stage 19 and stage 21(Hilly ITT). Dont get me wrong, he needs a teams(everybody does, even pogi. (actually think Pogi had the better team in tour 23 or at least equal.) I think Jonas will win between 8-11 Gts in his carrer. My guess at least 5 tours, 3 Vueltas and one giro, Pogi will win hopefully one last Tour late in his carreer. So 3 tours, 1 vuelta and 2 giros. I do think Pogi will try as his carreer goal to win every major race on tour including olympics and WC rainbow, and he will probably succeed. Pogi will become one of the best ever, maybe the best, since mercxk did in a time where it was easier to do(my opinion)and jonas maybe the best ever GT racer since he had to compete against the GOAT and still beat him so many times. Why do u think Pogi choose the giro. 1. he has not won it yet. 2. He knows he will probably not beat The GT s Jonas is in. He has never beaten Jonas in a GT 3rd week and probably never will. Thats my opinion, but time will tell and one thing is for sure. I have watched road race cycling since mid 1980 s and right now is in my opinion the most exiting times to be a cycling fan that i have experienced. We are so blessed with great talents and almost every race is a firework of aggressiveness, and opportunistic racing. I LOVE IT

    • WVA has broken his clavicle (AKA collar bone), and had already been operated for that by one of the best sports doctors in Europe, and has also broken/cracked 7 ribs, and cracked his sternum, but I've read nowhere that he has also broken his scapula (AKA shoulder blade).... It's bad enough as it is.

    • I've had broken collar bone and 3rd degree separation and 4th degree separation on shoulders but my broken ribs were by far the worst injury I have ever had from bicycle racing.

  • i've riden with a lot of busted bones. i rode in a cast for 6 weeks when I broke my schafoid. i've riden with a broken foot. but when i cracked my ribs in a race i was able to finish just fine but the next day i couldn't even bend enough on the bike to get my fingertips on the hoods. i couldn't breathe deeply at all. my guess for wout is tdf with a fiocus on the Olympics and worlds.

  • Instead of speculating on injured riders future prospective, I would prefer some relaxed commentary about their feelings and situation. I am pretty sure WVA isnt lying in the hospital wondering if he can make the Giro or not. I might be wrong. The focus on who can win want kinda loose perspective for me with such a crash.

    • I never once worried about how much pain I was in. I only wondered when will I be able to start training again and then what’s the next race I can make and be good at. My heart never once played a part in it except, I felt a bad for my wife cause I was not going to be especially happy for a very long time now. 😂🦋👊❤️

  • Small nuance on your fracture assessment- all broke bones are considered “fractures” but can be of varying severity (hairline, greenstick, spiral, comminuted, displaced vs non displaced). Always enjoy your commentary!

  • I had almost an identical injury, 4 ribs with 8 breaks and a shattered clavicle (no sternum) and was back at my construction job two days later. Took about 6 weeks to fully heal. There will be a lot of pain though.

  • I don't truly believe that a powerhouse like Mads has the ability to really hold back, but I do think that would be his best plan for Flanders. Using it as a warmup to Roubaix, which is his favorite race of the year, and which also fits him slightly better tbh. With Tadej and Wout not present, the group of big names is narrowed down, which can only be good for Mads' chances. It's obviously a shame that Wout crashed out like that. We all want to see the best riders compete in the biggest races. I'm just trying to see the good in the chaos here. I really want Mads to win it.

  • Such great technology with plating the collarbone. I couldn't move for a month when I spiral fractured mine in the 80's. (I also separated my AC joint & tore my deltoid 180° around my shoulder, but the collarbone was the hardest to heal.)

  • I honestly wouldn't be surprised if VA doesn't ride the TdF, but instead focuses fully on olympics, Vuelta and Worlds (+ maybe Lombardia with the climbing shape he has from the Vuelta and worlds). This changes less in the season plans, gives him more time to recover and lets hims focus on his remaining main goals instead of having to work for the team in the TdF.

    • You are right that a fracture = a break, you are wrong about the sternum… if you have ever witnessed a sternal saw chopping through it at the beginning of cardiac surgery, you would know this… perhaps you are thinking of the xiphisternum? This is the inferior bit but the sternum is most definitely a bone

    • They really affect your sleep and you never enjoy a good shower for sometime assuming there is no infection. Fitness is more affected by the impact of the crash its self. If possible always slide more during your crash and life will be uncomfortable for some time but you can still go to work. 👊🦋😂

  • Chris, a rib fracture is a broken rib. When you say “ anything more than a fracture (like a complete break…)” well a complete break is a fracture. As you well know, rib fractures are going to be unrelentingly painful for at least 2 weeks, before easing off more or less completely by 6 weeks. They are not easy to deal with as an athlete.

  • Chris, I’d love to see Pogi too but acknowledge unlikely now. Pogi believes Jonas can win a monument (to be fair he’s better placed than pretty much anyone) as they’re so long. RVV is hillier this year from further out. Visma have space on the team. His tactics would be simple (get rid of MVDP on the Kwaermont). He is unlikely to deviate from the annual plan BUT what an opportunity! Phenomenal marketing/exposure for Visma too….

  • Thanks for the video, interesting. Having broken ribs in a cycling crash myself (I'm an older rider) I was on the trainer after 2 weeks. Did not ride a bike outside for 8 weeks and did not return to Crits for 12 weeks. You Pro's are tough, anyone who says cyclists aren't tough is full of crap!

  • I have no doubt that WVA will recover. He got injured too during TDF ITT. He torn his upper thigh muscle (?) and he came back strong. But this time I think it's not the physical recovery that's will be difficult. But it's his psychological recovery. It's a bit blow to his morale. After, he has sacrificed his cyclocross season, reduced his program, opted to spend training in Tenerife, just to crash out before he raced his objective race that he has been gunning all years. That's a tough blow. Too bad, he doesn't have Roglic as his teammate anymore. Rogilc is the master of bounces back from huge set back. Giro? He might just use it as training ride for Olympics perhaps? I'm curious if VL will put him at TDF and he will ride domestique again for Jonas. I think, if he is in the Tour, I don't think he'll be the typical WVA we see in front Jonas in the mountain. His next goal is Olympics. So he isn't going to burn himself. That's probably the reason he wants to do the Giro instead (?)

  • Chris, thank you for your personal accounts, helping us imagine how long Wout might take to recover. I just wanted to say that I already knew how hard professional cyclists suffer on those hard climbs and I also knew that it takes an immense amount of willpower to go out and train every day no matter what else is going on in your life. But hearing how often you have broken your bones and even thinking about sitting on the trainer so shortly after multiple fractures that's just superhuman just like the speeds of the pros on the flats, just like the climbs and the times you manage. This sport is so demanding and brutal, huge respect.

  • 7 ribs means it was a very big impact. Sternum takes long to recover. Yes he can race the Giro with mild discomfort while still recovering, but why risk it? If he needs to be off the bike for more than two weeks, and then another two weeks of building up to normal training, his winter fitness build up is gone. Might still be able to challenge for some sprints, but not long and hard stages like the Giro has, so for me it makes no sense to go there. Tour it is, I'm certain.
