I think I know what's going on with the default position glitch. From what it looks like, whenever you enter an arena like rykard's and there's a lost grace inside of it, it starts a failsafe. I don't know if you'd need to have no bonfires before doing this, but it seems that if you have none except the one in that little area, a save quit will put you at the default position of the last area you were in. since you can't teleport to the lost grace for the rykard fight, you have to die in an area to return to it, but since the area you died in was the last place you've been, it teleports you to the default spawn location on a save quit. I've actually had a similar glitch happen when I tried to fight the ancestor spirit deer thing, where I rage quit and came back to find myself at the bonfire in mistwood (i didn't find any in the siofra river at the time, it was pretty grueling).
