Schalke vs. Dortmund is clearly the best known and biggest derby in Germany, but there are others which are possibly just as fierce, or in some cases even maybe more bitter rivalries. But they don't happen that often nowadays because the teams are not in the same leagues. Eintracht Braunschweig - Hannover 96, FC Bayern - 1860 Munich, HSV - St. Pauli, Eintracht Frankfurt - Kickers Offenbach, to name a few.

And yet people still say Bayern are Dortmund's biggest rivals. Plastic Dortmund "fans" that emerged after they beat Real Madrid.

A derby is match between two rivals either it's is in the same state(e.g. Man city vs man UTD , both clubs are from Manchester) or just two big teams rival each other(e.g. Barcelona vs Real Madrid, both teams are not the same state but they are 2 biggest club in laliga history)
Madrid vs Barca isn't a derby, it's just a big game. Espanyol is Barcelona's derby and obviously Atletico are Real Madrid's. Like you said a derby is two teams who are in the same city or geographically close to each other, Man Utd vs Liverpool isn't a derby because Everton and Man City exist but Newcastle Vs Sunderland (which are two separate cities) is a derby because there isn't a team in that city.
