  • I’m a 58 yr old Englishman who got upper and lower metal braces for the first time ever on 08/11/23. My orthodontist used 14NiTi wire but placed a powerchain on my lower brace in order to close a small gap on my very first appointment. Is this normal? I have not had any pressure let alone pain from my teeth after the first week (my cheeks have been cut up a fair bit though). So many people on the Internet seem to make such a fuss about braces causing terrible pain but I have had none, even with a powerchain fitted at my very first fitting appointment.

    • Hi! Thanks for watching my channel. I think it kind of depends? It's not something I"d routinely do but if the teeth were very flared to begin with from periodontal disease or another issue, I might be ok doing that to accelerate the esthetic outcome? But it's a one off- I probably don't mention. that in my video because people are very literal and they might just do it when it's not a good idea.

  • Hello! My orthodontist is very green, and I was surprised when she put a power chain on at my first adjustment today. Do you think I should question it?

  • Can you explain why you powerchain on niti? In residency we learn never to powerchain on niti wires, only on rectangular stainless steel wires. I would really appreciatie your opinion on this ❤ In practice I find that if I powerchain on niti wires, the teeth rotate in or the arch collapses because niti wires are so flexible. I’m very curious to know why you don’t use stainless steel archwires. Thank you so much ❤

    • We learned the same but they never really gave us a reason "why not"- it was in the course but then all the successful preceptor faculty told us it was BS and they all power chained on HEAVY niti that fills the slots (like 18x25/19x25) and once i started to do it I realized it was fine. No side effects. Don't do it on 16x22 or 018 though and ONLY do it if the wire is passive and you've checked the roots and they are upright and parallel. studies say SS has better sliding mechanics but I've done plenty AB testing and noticed nothing different. Also SS wires can bend when paitents eat something hard or crunchy and weird things happen.

  • How long does it typically take to reach the "smile ok" but bite refinement can be tweaked?
