Pregnancy Headache Clinical Examination - OSCE Revision - Dr Gill
When it comes to headaches, there is perhaps no clinical consultation more frightening to the medical student or junior doctor than the pregnant lady with a headache!
When it comes to pregnancy headaches, you are not merely looking after your patient, the mother, but also your secondary patient of the unborn fetus
It is vital that we exclude dangerous pathologies such as meningitis, pre-eclampsia and raised intracranial pressure, whilst also helping to narrow down the cause of the pregnancy headache - often migraines, tension headache or cluster headaches
But it is also vital that we keep in mind the rare causes which can cause a headache in pregnancy; subarachnoid haemorrhage, CVT, vasoconstriction and ischaemic CVA

Different medical schools, nursing colleges and other health professional courses will have their own preferred approach to a clinical assessment - you should concentrate on THEIR marks schemes for your assessments.

  • I could never get into the mindset of repeating questions of which I already know the answer to, so watching this, knowing that you are well aware of the answers to what you're asking but being able to still act it out for the camera is astonishing to me. And, of course, they are very interesting to watch, even if I have 0 plans to enter the medical field.

I was wondering, will you be making any videos relating to babies/child conditions such as bronchiolitis (which my little one has had twice recently), such as what helps or doesn't help and what causes it in the first place 😊 A child series would be super helpful

  • Excellent video and congratulations to you both on the birth of your son. Towards the end if my second pregnancy some protein was found in my urine however the Consultant was unconcerned and said that he probably had protein in his urine occasionally so it was nothing to worry about! Reassuringly everything continued as normal and I delivered a healthy baby boy a few weeks later.

  • I had a preeclampsia headache with my middle daughter. It came on suddenly and was massively intense. Luckily it happed during a stress test at my drs off so when I screamed, not meaning to, off of the sudden pain my dr was right there and after checking my blood pressure said lets go have a baby. She was very calm they gave me something to bring my blood pressure down which helped with my head. Not a situation I recommend. Oh my that hurt. That whole pregnancy was awful. 10 weeks of strict bed rest

    • ​@@DrJamesGilloh yeah. We had a C-section, she was frank breach anyway and upon examination I was already 4 centimeters dilated. I was 37 weeks pregnant. She is now a beautiful happy healthy 10 year old. The funny thing is a year later I got pregnant with my youngest and outside of a little bleeding in the beginning that pregnancy was so simple. Outside of getting bigger I wouldn't have known I was pregnant. Morning sickness was extremely mild and short lived. No emotional rollercoasters, no preterm labor. In fact I ended up being induced TWICE! I didn't want a C-section again so when the first induction didn't work at 40+3 weeks I convinced them to let me go home since my water hadn't broken. I was induced again at 42 weeks and that did it. So interesting how each pregnancy can be so very different. My first pregnancy was easy till it wasn't as well. I ended up going into preterm labor and having my oldest at 32 weeks. But yeah that headache was just awful. I knew what was happening, I was terrified of having a stroke. But my Dr was so calm.

  • Hi Dr Gill! First of all, sorry for bothering you in advance. I just wanted to ask if you could you make a small video talking about CBD and it's effects in parkinson patients. A family member is going through it right now, and many people suggested that CBD oil could help to manage the symptoms, since we are not very sure about it, would be helpful and also interesting!   • i wish i could find a Dr who actually cared to make sure i’m okay. i had debilitating headaches during my pregnancy. literally only got them during pregnancy and they’d hit me in the afternoon. i’d be in so much pain for hours and my OB was just like “oh yeah that’s normal drink water take tylenol”

  • Would it be possible to add short comments about what the different questions and procedures aim at checking for? I am absolutely not wanting to add to your workloads, and I imagine that these videos are primarily for medical students and practitioners, but I still wanted to ask because I would love to know why you ask about whether her headache gets worse when she's tying her shoes, or what frothing urine might indicate. Again, this is not at all something you need to do anything about. It's just a thought

    • @DrJamesGill My mum was getting severe headaches, and the first hospital she was sent to sent her home again, and it turned out that it was a ruptured brain aneurysm. There was nothing they could do at the second hospital she went to.

  • 4x2 paracetamol for a pregnant woman? 😮 I know in the UK people use paracetamol for everything they eat it as peanuts but... Isn't it known as the effective dose very close to the hepatotoxic dose? 🤔

    • Not really no. Paracetamol at that dose is safe as houses, as someone can use that dose for YEARS and no problems.

    • I think of it as speed limits. Doing 40mph in a 40mph, is completely fine. Doing 50mph in a 40mph and I’m in a lot of trouble

    • @@DrJamesGill But there are less dodgy substances around, aren't there? For instance, I personally use metamizole (marketed here as Algopyrin) for less severe pain and fever, works fine. It's a non-NSAID and it's been around for nearly a century now, has to be safe. Mind you, I am very (over--?) cautious about pharmaceuticals, resort to them only if inevitable and in the smallest effective dose.
