Always knowing what to say usually happens outside of yourself and when you're giving the person you're speaking to your full attention. Not thinking about things like "what should I say next", "was that the right thing to say" and etc. It's literally just getting outside of your head which does require a little bit of getting used to but anyone can learn if you practice it often despite it being a little awkward in the beginning.
