  • Dear Zach, I wanted to express my gratitude for your in-depth coverage of a highly significant medical issue. I just finished watching your comprehensive 3-hour video, and I'm truly grateful for your informative and precise summary. I would like to contribute an essential point regarding the complex topic of elevated UNa in siADH. One might assume that UNa levels should decrease as the kidneys attempt to reabsorb sodium in response to elevated water levels, aiming for a balance. However, due to the heightened water levels and the relatively euvolemic volume status, the RAAS becomes inhibited. Consequently, natriuresis increases, resulting in a rise in UNa levels. I hope this information aids in a better understanding of this crucial subject. Greetings from Germany. Janis

  • Great presentation that helped me understand. I know of a person with this condition of hyponatremia stemming from dysfunction of her pituitary gland. She lives on Gatorade, but regularly ends up in the ER receiving sodium and or potassium drip to regulate her. She drinks a lot of fluids, yet doesn't eat much food, so she has frequent constipation.

  • Great video Zach, I really liked the lecture. There is just one point where I am unsure if you are correct. If a patient is hypokalemic, its mainly their intravasal potassium which is low. It can be true that cells can also be potassium depleted in a case where they have previously tried to correct the intravasal potassium with a shift of potassium. However, it is not the case that cells actively release sodium 'in exchange' for potassium; the exchange process is not a direct 1:1 exchange of potassium for sodium. The sodium-potassium pump works continuously to maintain balance by transporting more sodium out of the cell than it brings potassium in, which is crucial for maintaining the cell's resting potential. This process requires energy (ATP) and is an example of active transport. Therefore, administering potassium can lead to cells taking up more potassium, and the activity of the sodium-potassium pump can contribute to an increase in the extracellular sodium level. However, this is part of the normal function of the pump and not necessarily a direct response to potassium supplementation. Please correct me if I am wrong. Post transphenoidal resection of the pituitary macroadenoma, the patient gets grandmal seizure serumNA+ 99, loading done but the results are above 145, thyroid function test lower, she has. DI presentation (insidious).

  • I love this channel so much!!!! I am hyponatremic, hypoglycemic, can't digest a bunch of stuff. It's central nervous system failure due to having EDS with 9/9 points. Lots of autonomic disfunctions, like POTS. It's the pits! 🥺
