That just happened twice .. on 4/7/1999, in a match between Argentina and Colombia at Copa America 99, Martin Palermo caused and took all the three penalties in the match but he missed them all .. on the opposite side and on a match between Brazil and Argentina at the World Cup 2006 qualifications, Ronaldo caused and scored three penalties (four if you count the annulled one) Both players written down their names on Guinness records and Marcelo Bielsa and Zanetti were the witness on both occasions
The Phenomenon (Fenomeno) and Brazil icon, on 3-6-2004 got all the three penalties by himself and scored four goals out of it!!
Ronaldo took the penalty himself and sent his kick low to Pablo Cavallero's left only for it to be disallowed because his team mates were inside the area. For the retake, he converted an almost identical effort.

The backlash from the press was severe; Palermo now held the world record for the most penalties missed by one player in an international game.
