  • There is a strong argument to approach all degrees of thermal burn injury using the same first aid process. That is, all will benefit from water cooling including full thickness (3rd degree) burns as such burns rarely occur in the complete absence of superficial and partial thickness burns also being present. Thus realistically, more severe burns should be seen as "composite" burns (i.e., containing 1st, 2nd and 3rd Degree injury) rather than just third degree burns in isolation. As the ongoing aim of treatment includes reducing the extent (%) of the burn caused by burn progression, it necessarily requires all thermal burns should be cooled. Fears about cooling induced hypothermia remain an impediment to effective care. New priorities for emergency carers who attend must be a focus on patient warming. Using this approach means the primary carer (family member, workmate etc) provides the cooling and early covering if time permits before ambulance arrival, while the paramedics focus must be to address warming, shock and pain management, secondary injury etc. This provides a continuum of care right through to the ED door.
