  • I spent years in pain and agree with everything here except the orthotics - any architect will tell you that you NEVER support an arch from underneath , the strength comes from above ! Barefoot shoes were the answer for me ( along with toe spacers and foot exercises ) being very careful to select shoes that are extra wide with a wide toe box , my shoes all have enough room to wear Correct Toes underneath and my back and knees and hips are all feeling the benefit as well as my feet

    • How were barefoot shoes the answer? Becuz u could wear the toe spacers with them? I wear Birkies Have since 1990 (95% of the time) My feet have high arches I wear out soles on outside (not pronating) I can’t go barefoot, I pay for it; I should wear shoes always but I hate them in the house-I do put on shoes to cook or clean. I have plantar fasciitis flares too so that motivates me to wear shoes. I’m to the point I can only wear Birkie sandals as there’s just no shoe wide enough for my bunion as my feet are already wide-always have been.

    • Its absolutely nuts your not going to change an anatomical deformity with this nonsense. You canhelp the discomfort but a permanent fix takes a skilled Dr . Not everyone fuses the joint their are multiple procedures to fix multiple types of deformities. And ethical people do the proper procedures for all types of problems.

  • Do you recommend Voltaten ? I use it on my bunionettes and it works well. Also as PT I do recommend massage and Pilates more than yoga. Core strength and alignment of the spine into neutral changes weight bearing significantly and is the whole focus in Pilates. ( and no crazy moves). Kinesiotaping helps a lot and regular flexibility classes. When I added flexibility classes to my business during Covid I got the best feedback on how pain in the foot, ankle and knees changed.

  • I run a lot and cycle but recently I noticed a bunion on my left foot after changing my shoes. I have flat feet too. I use to wear ASICS with midsole support but then I purchased a cheaper pair of ASICS which had no midsole support. Also, I bought a new pair of slippers which was half a size smaller then my foot size and forced my feet to stretch them to fit my feet, wearing narrower socks. Also, I haven’t been stretching a lot lately or doing any lower body strengthening exercises. I’m now stretching more and doing stretches/strengthening exercises for my feet/ankles and some glute strengthening as well. Walking around barefoot in my house on wooden laminated floors. Wearing toe socks when I sleep now. I’m considering purchasing correct toes toe spacers and Altra running shoes with cushion support, zero drop heel support and wide toe box. I’m just not sure should I buy shoes with midsole support or just work on strengthening my arch on my flat feet?

I didn't have pain in my 20s but it started to hurt a little more as I got older, just an achy sore inflamed feeling at that joint. But I also hated how they looked, how my feet fit into shoes, etc. I only wish I had done my surgeries sooner. The first few weeks of recovery really suck, but I'm so happy I did it. I had my bunions since I was a kid, and I can't believe I waited this long to have straight toes.

They didn't do any stretches; she just touched the side. I was terrified, so I left. I keep wondering why surgery was the first suggestion. I haven't gone back, and they've been calling since. I've been exercising and stretching my toes, but it still hurts when I press the side, although the toes still look straight. I've watched many videos and am considering seeing another doctor. What would you suggest I do? I use to wear heel a-lot but I stop wearing them for a while because i want to give my feet a break.

    • Personally, something like this may help with a slight amount of pain relief, but the key is fixing the underlying biomechanics: I would always recommend initial protection & offload(shoes & insoles), home physical therapy, massage, then exercises: kzhead.info/sun/ja1po6WGkHZ8aYU/bejne.html Always follow with your hip specialist as well for evaluation!

    • Parasites (linked to all autoimmune conditions...& bunions...& arthritis) absolutely LOVE joints. They camp out & raise families there-- big toe, jaws, wrist, knees, hips. Ask anyone who's worked in a slaughter house or butchered hogs...or gotten rid of their own rarely diagnosed roundworms, tapeworm larvae, flukes, filarial worms of any sort. Mine had become systemic...heart, lungs, liver, sinus cavities & spinal fluid, plus gut. My bunions developed & became severe. As the parasite load decreased, i could use the toe separators mentioned here & get "the gaps" back, & no pain. A USA stool test will almost never diagnose parasites, because they have no reason to let go & exit via poop. I learned how to diagnose & treat & change lifestyle thru an MS group that Pam Bartha started. It's based on reasearch by parasite & Lyme doctor folks like Dale Klinghart & a doc whose name i forget who finds parasites in the blood or spinal fluid of 100% of autoimmune patients tested. If bunions & autoimmune & jpint pain coincide, parasite involvement is almost certain.

  • I've been wearing wide rounded shoes all of my life, I hate to have my toes squeezed, and I also love comfortable thick chunky shoe soles. I can't wear anything flat, like balerinas, or stilletto's. But what causes my bunion to hurt and if I don't stop it I feel like it's going to get worse. It's all good all day long even though I'm on my feet all day, walking with dogs and doing house work and nanny stuff, playing and dancing with kids, I never sit during the day. But my bunion doesn't hurt until I drive for too long. Sitting in the car after work for over an hour, as I get into a rush hour, causes so much pain and irritation for me. The thing is, I used to drive manuals in Europe and I was using both feet the same, for having 3 pedals, and using my left foot for the clutch and slowing down with the manual stick. Driving automatic in the USA caused me to have all the effort on my right foot, while the left one is not being used at all. It causes pain and irritation and frustration halfway home and it doesn't stop. It's the only reason I feel this problem. I guess I need a car with the manual stick and to get a job closer to my home lol.

I'll never try a toe separator again. I've found that Walking for 20-30 mins a day most days helps the orthotics and shoes to strengthen the arches which helps straighten the big toe a lot. This is a good video except for the recommendation for toe spacers. I'll personally never use them again I curse the fact I ever tried them.

  • I was shocked and intrigued to find acupuncture as a treatment recommendation for pain by a doctor. How did this make the list? Have you read the results of the studies you cite or just the summaries? Was this included because it makes people feel good? At 1:24 you stated " ... a lot of surgery being pitched, a lot of fake remedies that don't work. So everything I talk about here is going to be science backed and I'm going to talk about whether it works or not ..." and stating at 3:35 "It's [acupuncture] actually been well-studied that there are some benefits." Well, you're correct, acupuncture has been well studied but it's failed to demonstrate it's clinical viability. If this video was science based, I can't find the evidence within the video to support your position. The Dec '21 study from Trinh (PMID: 35003508) you cited states "... Due to clinical heterogeneity for all outcomes, quantitative analysis was not appropriate. Our Grades of Recommendation, Assessment, Development and Evaluation rated all outcomes to have either low or very low quality of evidence. ..." Low quality of evidence? That's the support for the recommendation? Clinical research can never prove that an intervention has an effect size of zero. Rather, clinical research assumes the null hypothesis, that the treatment does not work, and the burden of proof lies with demonstrating adequate evidence to reject the null hypothesis. After decades of research and well more than 3000 trials, acupuncture researchers have failed to reject the null hypothesis, and any remaining possible specific effect from acupuncture is so tiny as to be clinically insignificant. Madsen (BMJ. 2009;338:a3115) looked at 13 trials with 3025 patients, in which acupuncture was used to treat a variety of painful conditions. The meta-analysis has found that there may be a small difference between sham and real acupuncture, but clinically insignificant. Vickers (Arch Intern Med. 2012;172:1444-53) a meta-analysis for 29 randomized controlled trials, with 17,922 patients found that real acupuncture was better than sham but by a tiny amount that lacked any clinical significance. Again there was a somewhat larger difference in the nonblind comparison of acupuncture and no-acupuncture, but again it was so small that patients would barely notice it. The most parsimonious explanation is that the positive studies are false positives or even regressions to the mean. False positive results are common for multiple reasons, including bias and low prior probability. This recommendation of yours perplexes me.

    @caiusscipio6390@caiusscipio639010 ай бұрын
    • I 100% love the work you put in. Specifically I have listed these because I frequently have patients that get acupuncture and anecdotally report good results. I can see why specifically people do not get great results in the studies, as this is not a direct treatment, instead acupuncture, physical therapy and massage all would serve to help the underlying lower extremity mechanics and thus indirectly help the bunion. Acupuncture directly on the bunion and foot is probably not helpful, I fully agree. I don't see how you can properly run a study including these downstream variables, but overall people that try acupuncture seem very happy, even though things like physical therapy and massage would be no doubt excellent and potentially easier options. At the same time, it was meant to be near the bottom of the list for a reason 😄

    • @@DrTomBiernacki Thank you very much for taking the time to respond and expanding on your thoughts. I appreciate it! I agree that it'd be hard to develop a study looking at the downstream effects of acupuncture when it hasn't been able to demonstrate a there there, yet. It'd be easy for someone to be prescribed physical therapy, massage, and even surgery and then go off and do acupuncture on top of that. Confirmation bias can be quite high when a good outcome is had. I can also appreciate this being at the bottom of the list. I was puzzled why it would even make a "scientific list" given the available information. But I think I understand your position a bit better now. Thank you kindly for taking the time.
